This waiver describes the risks of participation in F4 Flag Football programs and the acknowledgements a parent or legal guardian makes when registering a child. A parent or guardian accepts this waiver during registration.

1. Assumption of risk

Flag football and related activities involve inherent risks, including the risk of physical injury. By registering a child, the parent or guardian acknowledges and accepts those risks on the child's behalf.

2. Release and hold harmless

To the fullest extent permitted by law, the parent or guardian releases F4 Flag Football, its staff, coaches, and volunteers from claims arising out of ordinary participation in the program, except for gross negligence or willful misconduct.

3. Emergency medical authorization

If a parent or guardian cannot be reached in an emergency, F4 may seek appropriate emergency medical care for the child. Families should keep emergency contacts and medical notes current in their account.

4. Photo and media

Photos of a child are collected and used only with parental consent, as described in our Privacy Policy. Consent can be withdrawn at any time by editing the child's profile or contacting us.

5. Parent/guardian acknowledgement
